Discussion Question 2: Finding Errors in Drug Orders

Consider the following dosage amounts written on a selection of drug orders. Some of these are written correctly, and some of them contain mistakes.

Use THE LECTURE ONLY (or the ISMP List of Error-Prone Abbreviations, Symbols, and Dose Designations that is linked to in the lecture) when answering these questions. Do NOT use other sources on the internet or AI. If you use other sources outside the lecture, it could lead to receiving a ZERO on this assignment.

For each of these dosages:

1) identify which system is being used to write these dosages (or if it is unclear which one, explain which detail of the way it is written makes it unclear);

2) If it has been written correctly, indicate that. For each dosage that you believe has been written incorrectly, explain:

A) how it should be written correctly within the system in which it has been written (regardless of whether that system is still officially in use);

B) what is wrong with the way that it is currently written (there may be more than one error in some of the incorrectly written dosages--be sure to identify ALL of the potential errors);

B) which source you are using (the lecture or the ISMP list) to identify each error (you can simply write "lecture" or "ISMP"); and

C) explain how this error or these errors could lead to a patient being given an incorrect amount of medication.
